AMHL Incident/Complaint Form (Ancaster Minor Hockey)

This form is to be used by AMHL members to submitt a formal written complaint regarding the behaviour and/or actions of another AMHL member. Upon completing this form, your complaint will automatically be submitted to to our Incident Resolution Committee (IRC) for review. Allow a minimum of 72 hours for the IRC to contact you directly. After all investigations and meetings, the IRC will send it's recommendations to the AMHL Board of Directors.
