To access the player on-line evaluation, please follow the instructions below:

·         Please make sure your pop up blocker has been disabled.

·         Click Link: www.impacttestonline.com/CMRG

·         Select the Province of "Ontario" and then “Launch Baseline Test”

·         Input the following access code: 76B1811CD0

·         Select Language

·         Click “OK” on the disclaimer page

·         Choose “English” for the unit of measure and click “Next”

·         Click “Next” again to begin the testing

·         On the Sport and Health History page, under “School / Organization”, select your team from the drop-down menu. 

·         Complete the test
